Recovery Nurse

The Recovery Nurse is to serve selected clients who are the claimants under the Recovery Program. It is to adopt a holistic approach in the delivery of physical, psychological and social support via phone contact and to align various support services available. It fosters a compassionate caring concept to ease the disease burden, better coping and to improve the quality of life of an individual.
Roles and responsibilities:
• Be the key stakeholder in meeting the individual's health needs and identifying health gaps/services that the individual requires during the recovery process.
• Establish rapport and support for the claimant through establishing a therapeutic relationship involving physical, psychological and emotional health components.
• Assess the health needs of the individuals with ongoing evaluation and re-assess to prioritize corresponding care for recovery.
• Collaborate and arrange health services to achieve the best health outcome of the individuals.
• Provide evidence-based practice on health information and resources to individuals, ensuring appropriate care and advice are delivered.
• Promote evidence-based health concepts and adaptation of healthy lifestyles for better health outcomes.
• Ensure a compassionate, caring and safe practicing environment for claimants, adhering to the professional standard and code of ethics.
• Maintain claimants confidence by ensuring all interactions are conducted with strict confidentiality and maintaining a trusted, professional relationship.
• Produce accurate and truthful documentations on all claimants contact and progress.
• Maintain precise handover amongst team member and other service provider for a seamless care delivery.
• Ensure the compliance of local nursing authority registration and requirements set forth in local law or legal compliances.
• Ensure up-to-date local practicing certificate/licensing and maintain continuous professional nursing practice & education to uphold national quality standards in nursing practice.
